Waiver to FDA Medical Evaluation Requirements

This is the waiver to the medical evaluation requirements when purchasing hearing aids. Please read carefully.

You, the prospective hearing aid user, must be 18 years of age or older.

The hearing aid user should get a medical evaluation prior to the purchase of hearing aids, as this may not be the best solution to your hearing problem. We do not encourage you in any form or shape to bypass the medical evaluation. The exercise of the waiver is not in your best health interest. Please see FDA Notice for more informaiton.

I have been advised by AmperorDirect.com that the Food and Drug Administration has determined that my best health interest would be served if I had a medical evaluation by a licensed physician (preferably a physician who specializes in diseases of the ear) before purchasing a hearing aid. I do not wish a medical evaluation before purchasing a hearing aid.
